What should I expect at my first appointment?
Do you take insurance?
We do not accept insurance, but offer a superbill for your services that you can submit to your insurance for possible reimbursement.
How often will I follow up with the provider?
The first follow up appointment will be within 2-4 weeks depending on how the tests you are doing and how quickly we receive the results. The follow ups are typically every 3-4 months thereafter.
What is the cost to become a patient?
The office works based on a membership program. The initial fee at this time to become a patient in the practice is $895. The monthly fee is $195, which will be charged to your credit card on file on approximately the 15th of each month.
What is covered under the membership?
The visits with Dr. Taylor and/or our NP or RN are covered under the membership. Testing, procedures, medications, and supplements are not included. The cost to see our functional nutritionist is not included.
Can I cancel my membership?
Yes. You must submit your request in writing. Your membership will be cancelled 30 days after we receive your notice.
